Relapse Prevention Planning is part of after-care, (when the client returns from rehab) and is critical to long-term sobriety.
I am available to work with individuals returning from treatment or anyone concerned that they are at risk of relapse.
I will guide the individual through a process to become aware of their high-risk thoughts, feelings and behaviors so they can see more options and make better choices. This specialized technique allows the addicted individual to develop a Personalized Relapse Prevention Plan that makes sense to them.
